Toward Maximum Independence (TMI) is a nonprofit organization that has been transforming the lives of children and adults with developmental disabilities since 1981 and the deaf and hard of hearing since 2000. TMI is committed to the full inclusion of people with developmental disabilities into all facets of community life.

Toward Maximum Independence Community Living Services (CLS)
Supported Living Facilitator -Full Time 
Hours are Monday through Friday from 2pm-8:30pm along with Sunday 5pm-8:30pm. 

The Facilitator must have a good understanding of, and believe in, the philosophy of Supported Living. They must be willing to work well with the other members of the support team. They will treat the person supported, and all members of the planning team, with dignity and respect at all times. The DC is under the direct supervision of the CLS Case Manager and Director of Community Living and indirect supervision of the Associate and Executive Directors. 

The qualified candidate must be teaching independent living skills to individuals which may include meal preparation, meal planning, money management, problems solving, attending medical appointments and accessing community resources.

A belief that all adults can live in the community, in a home of their own given the supports they need to be successful
Proof of at least 18 years old and minimum three years driving experience.

Ability to perform the essential functions of the position as determined by the needs of the individual to be supported
Current certification in CPR and First Aid, pass all requirements for personnel, drug and criminal background screenings, vehicle inspection, proof of required vehicle insurance, valid California drivers license, clear driving record, pass TB test and attend paid training.

Essential Functions: The facilitator must be willing to listen to the person supported and adjust the schedule according to needs.
The facilitator will spend time with the person supported working on specific goals and objectives, connecting to the community, developing relationships and natural supports, exploring new adaptations, and participating in meaningful activities and organizations. 

The facilitator will communicate regularly and as needed with the planning team members to keep them updated. They will inform the coordinator of all emergencies, unusual incidences, any signs of illness, schedule changes, scheduled meetings, contacts with family members, and contacts with SDRC. This can be done in the form of progress notes and by phone if warranted.
The facilitator will complete all necessary paperwork for TMI and Regional Center and submit the paperwork on time to include time sheets, schedules, incident reports, progress notes, and Pinnacle.
Provide individualized support at home and in the community based on the needs and schedule of the person receiving support 
Participate as a member of the planning team and facilitate the development of a circle of support, both paid and unpaid, to build relationships with family, friends, neighbors and other community members
Collaborate with other members of planning team to develop and implement Supported Living Plan 
Assist individual to be an active member of his/her community by identifying preferred activities based on individual's choice/interests.

TMI benefits include medical, dental, sick & vacation, other insurances, 401k matching retirement plan and holidays with consistent schedule of 30 hours a week eligibility. Mileage is paid as needed.

This position will become benefit eligible with additional hours equal to 30 hours per week.
