Description
Position Summary:
This grant-funded position focuses on providing comprehensive diabetes education and care to patients with Type 1, Type 2, gestational diabetes, and individuals at high risk of developing diabetes. Education is delivered via telephonic communication, video teleconferencing, and in-person sessions, which may require travel to multiple villages within the YK Delta Region.
The RN Case Manager/Clinical Diabetes Educator coordinates with village clinics, hospital staff, and the interdisciplinary team-including dietitians and pharmacists-to promote diabetes standards of care, maintain the Diabetes Registry, and ensure compliance with grant requirements. The Educator also participates in community outreach activities such as Diabetes Support Groups, media campaigns, and special events, while supporting program planning, goal setting, and partnership with tribal leaders.
Key Responsibilities:
• Provide individualized diabetes education and care via phone, video, and in-person visits.
• Order labs and document patient education in the electronic health record according to hospital policies.
• Collaborate with village clinic and hospital staff to promote diabetes care standards.
• Assist Diabetes Coordinator with maintenance of Diabetes Registry and grant compliance.
• Participate in community outreach including support groups, media campaigns, and special events.
• Work closely with dietitians and pharmacists to ensure comprehensive patient care.
• Participate in program planning, departmental goal setting, and corporate initiatives.
• Travel to villages as required and perform other duties as assigned.
Required Qualifications & Skills:
• Completion of a nursing program from an NLN-approved institution.
• Associate's Degree in Nursing required; BSN or MSN preferred.
• Valid Registered Nurse license in good standing in the State of Alaska.
• CDCES (Certified Diabetes Care and Education Specialist) certification must be obtained within 2 years.
• Two years of clinical health experience preferred; new graduates with relevant licensure may be considered.
• Basic Life Support (BLS) certification preferred.
• Experience collaborating with multiple disciplines for project completion.
• Knowledge of diabetes, cardiovascular disease, and public health.
• Proficiency in PC computers and diabetes care equipment (glucometers, scales, etc.).
• Ability to work in office, hospital, and village clinic settings, including travel via small plane.
• Strong cross-cultural communication skills; bilingual (Yup'ik/English) preferred.

Qualifications:
• Basic Life Support (BLS) Certification
• Advanced Cardiovascular Life Support (ACLS)
• National Institutes of Health (NIH)
• Sound knowledge of anatomy, physiology, pathology, and medical terminology.
• Active unencumbered Registered Nurse Alaska state license
• Graduate from a fully accredited Registered Nurse program
• One year of work experience as Registered Nurse
• One year of EMR experience
• Board Certified

• Participates in relevant continuing education programs.
• Knowledge of and dedication to the highest patient care and provide the utmost service excellence to all patients.
